Job description

Provide vaccinations, screenings, and counseling on healthy habits such as diet, exercise, and smoking cessation. Chronic Disease Management: Manage and monitor patients with chronic health conditions such as diabetes, hypertension, asthma, and heart disease. Provide ongoing medical care, medication management, and lifestyle interventions to optimize disease management and prevent complications. Acute Care: Evaluate and treat patients with acute medical problems such as infections, injuries, minor trauma, and respiratory illnesses.
