Job description

Preventive Care: Conduct routine health screenings, physical examinations, and assessments to identify risk factors, detect early signs of illness, and promote preventive measures. Offer vaccinations, immunizations, and health education to prevent disease and maintain wellness. Diagnosis and Treatment: Evaluate patients' medical histories, symptoms, and diagnostic test results to diagnose and treat acute and chronic illnesses, injuries, and health conditions. Prescribe medications, therapies, and treatment plans based on evidence-based guidelines and best practices. Chronic Disease Management: Manage chronic health conditions such as diabetes, hypertension, asthma, and arthritis through ongoing monitoring, medication management, lifestyle counseling, and referrals to specialists when needed. Coordinate care and support patients in self-management strategies to improve health outcomes. Acute Care Services: Provide timely assessment and treatment for acute medical conditions, injuries, infections, and minor emergencies. Perform minor procedures, wound care, and suturing as needed to address acute healthcare needs in outpatient settings. Health Promotion and Education: Educate patients and families about healthy lifestyle choices, disease prevention, nutrition, exercise, and stress management techniques. Offer guidance on smoking cessation, weight management, and substance abuse prevention to promote overall health and well-being. Women's Health and Reproductive Care: Provide gynecological exams, contraceptive counseling, prenatal care, and reproductive health services to female patients. Perform Pap smears, breast exams, and screenings for sexually transmitted infections (STIs) to promote women's health and wellness. Men's Health and Sexual Health: Address men's health issues, including prostate health, erectile dysfunction, and reproductive concerns. Offer counseling, screenings, and interventions related to sexual health, fertility, and reproductive function in men. Pediatric Care: Provide comprehensive pediatric care to infants, children, and adolescents, including well-child visits, immunizations, growth monitoring, and developmental screenings. Assess and manage common pediatric conditions and refer to pediatric specialists as needed. Geriatric Care: Care for elderly patients by addressing age-related health concerns, chronic conditions, mobility issues, and cognitive impairment. Conduct geriatric assessments, medication reviews, and advance care planning discussions to support aging patients and their families. Coordination of Care: Collaborate with specialists, allied health professionals, and community resources to coordinate comprehensive and integrated care for patients with complex medical needs. Facilitate referrals, consultations, and follow-up care to ensure continuity of care and optimal health outcomes. Electronic Health Records (EHR) Documentation: Document patient encounters, medical histories, assessments, and treatment plans accurately and thoroughly in electronic health records (EHR) systems. Maintain confidentiality and privacy of patient information in compliance with HIPAA regulations. Professional Development: Engage in ongoing medical education, training, and certification to stay current on medical knowledge, clinical guidelines, and best practices in primary care. Pursue board certification, specialty training, or continuing medical education (CME) credits to enhance clinical skills and expertise.
