Diagnosis and Treatment: Diagnose and treat a wide range of acute and chronic medical conditions, illnesses, and injuries. Prescribe medications, treatments, and therapies as appropriate to manage symptoms, promote healing, and improve patients' health outcomes. Preventive Care and Health Promotion: Provide preventive healthcare services, screenings, and vaccinations to prevent illness, detect health problems early, and promote overall health and wellness. Offer lifestyle counseling, diet and exercise recommendations, and smoking cessation support to help patients maintain healthy habits. Chronic Disease Management: Manage and monitor chronic medical conditions such as diabetes, hypertension, asthma, and heart disease. Develop individualized care plans, medication regimens, and self-management strategies to control symptoms, prevent complications, and improve quality of life. Emergency Care and First Aid: Provide emergency medical care and first aid for acute injuries, accidents, and sudden illnesses. Stabilize patients in emergency situations, administer life-saving interventions, and coordinate transfer to specialized medical facilities as needed. Referrals and Coordination of Care: Refer patients to specialists, healthcare facilities, or other healthcare providers for specialized diagnostics, treatments, or procedures beyond the scope of primary care. Coordinate and manage the continuity of care for patients receiving multidisciplinary or specialized medical services. Health Education and Counseling: Educate patients and their families about health conditions, treatment options, medications, and preventive measures. Provide counseling on lifestyle modifications, stress management techniques, and disease prevention strategies to empower patients to take control of their health. Medical Records and Documentation: Maintain accurate and up-to-date medical records, documentation, and patient charts for all patient encounters. Record medical histories, examination findings, diagnoses, treatments, and follow-up care plans in electronic health records (EHRs) or paper-based records as per regulatory requirements. Collaboration and Communication: Collaborate with other healthcare professionals, specialists, allied health providers, and support staff to deliver comprehensive and coordinated patient care. Communicate effectively with patients, families, caregivers, and interdisciplinary teams to ensure continuity of care and patient satisfaction. Quality Improvement and Patient Safety: Participate in quality improvement initiatives, patient safety programs, and clinical audits to enhance the quality of care and patient outcomes. Identify opportunities for process improvement, implement evidence-based practices, and monitor performance indicators to optimize healthcare delivery. Continuing Medical Education: Engage in continuous learning, professional development, and continuing medical education (CME) activities to stay updated on medical advances, clinical guidelines, and best practices in primary care.
