Health Promotion and Disease Prevention: Educate patients about preventive healthcare measures, healthy lifestyle choices, and disease prevention strategies, empowering them to take control of their health and well-being. Chronic Disease Management: Manage chronic medical conditions such as diabetes, hypertension, asthma, and arthritis, providing ongoing monitoring, medication management, and lifestyle counseling to optimize patients' health outcomes. Acute Care Management: Manage acute medical conditions, injuries, and emergencies, providing timely evaluation, stabilization, and treatment to address patients' immediate healthcare needs.
